CHEER FORMS

Please print and fill out the attached forms and bring them to Uniform Fitting.

You will also need your child's 4th quarter Report card(print it as soon as school ends, the district takes them down after a couple of weeks).

Please get a head shot picture of your child, and a copy of their Birth Certificate.

FOOTBALL FORMS

Please print and have your child's doctor fill out the attached medical form. This form will need to be uploaded to National Sports ID (NSID), which is a grade and age verification system we use. You will receive further information via email on this platform as we get closer to the season.

Players will also be required to submit a copy of their fourth quarter report cards to NSID. Please remember to print out a copy as soon as report cards are released as the district shuts down the site for several weeks over the summer.
